SECURE THE AIRCRAFT
- Aircraft Checklist Complete.
- Request emergency services as needed.
- CVR/FDR CBs pulled as required.
- Safety of Crew
- If time and circumstances permit:
- Call MX (760) 585-9672 and write up discrepancy (include any CBs pulled)
- If Evacuation required: Take phone, passport, and wallet if able.

IF IN A FOREIGN COUNTRY
Contact US Embassy/Consulate
OBTAIN MEDICAL CARE AND REST FACILITIES
- Keep Crew Together
- Obtain Medical Assistance
- Immediately ensure hospitalization and/or medical treatment for crew.
- Hospital may be required due to injuries or to escape media attention.
- Specify who can visit, – Generally only Family, Crew, and Latitude 33 Representatives.
- Obtain Rest Facilities for crew members away from scene.
- Advise Company / FBO / Family of Location.
POSTINCIDENT ALCOHOL AND DRUG TESTS (FAR 135)
- DO NOT consume alcohol or drugs until ALL required testing is completed.
- Drug test to be completed within 32 hours.
DOMESTIC
You have the right to question under what jurisdiction the request is made and to seek advice and counsel prior to submitting to test, however, refusal to submit to required Alcohol/Drug test will result in FAA Emergency Revocation of all Certificates.
INTERNATIONAL
You may be required to submit to Post Incident/Accident Alcohol/Drug Test in country of Incident/Accident as well as immediately upon returning to the U.S.
COOPERATING WITH AUTHORITIES
General Guidelines (Domestic/International):
- Avoid Oral/Written Statements without L33 representation:
- Keep responses short and factual.
- Avoid conjecture, hindsight, speculation, or judgement
- Sample written Statements are available in the L33 ERP.
- You may show your certificates if asked, but DO NOT relinquish them.
- Do not talk to anyone from the Media or Press.
- You may be in a state of shock and not realize it.
- When in doubt, seek L33 Legal Advice.
DOMESTIC
Law Enforcement
There is no obligation to make a statement. Should authorities want to charge you with an offense, you have the right to remain silent until you have consulted with your lawyer.
FAA
There is no obligation to discuss the incident/accident with representatives of the FAA until served with a subpoena. DO NOT TALK TO THE FAA WITHOUT COUNSEL. You MUST show the FAA your certificates if asked, but NEVER relinquish them.
NTSB
You are obligated to aid the NTSB in their investigation of an Incident/Accident, but in a reasonable manner, time frame and with legal representation. You have no legal obligation to take an NTSB-requested drug or alcohol test. This is not to be confused with Company directed DOT post-accident/incident drug and alcohol testing which is mandatory.
INTERNATIONAL
- Have a cooperative manner.
- Only if requested, provide your certificate to authorities for examination. DO NOT allow authorities to permanently retain any certificate.
- Only if requested, provide Oral/Written statement (keep copies.)
CRITICAL INCIDENT ON-SITE RESPONSE TEAM
Latitude 33 Aviation will dispatch an On-Site team to report to the incident commander (Police or Fire) or NTSB IIC if on the scene, to offer assistance. Action that requires completion include:
- Ensure the proper care of all survivors.
- Make sure that the event scene is secure.
- Establish and maintain communications with the Emergency Operations Center.
- With the permission of the IIC, gather, inventory and store the baggage and cargo.
- Provide technical support for all investigations.
- Support the coroner regarding the deceased.
- Safeguard and oversee the disposal of the aircraft wreckage.
- Support insurance company activities in coordination with the company’s Risk Management Representative.
- Support environmental impact assessment and cleanup since this can also be an EPA event.
- If approved, communicate with the media and other outside organizations at the scene.
- If requested, photograph the event scene.
CONTACT YOUR FAMILY
- Let family know location, contact information, and conditions.
- Release only relevant details of the event.
- Give family members L33 contact information.
- Advise family against speaking with NTSB / Authorities / Media without L33 Legal Representation.
The NTSB may contact family to arrange an interview to discuss the Crewmember’s activities during the days immediately prior to the incident/accident as well as any health/financial, social life events that may have impacted a Crewmember’s performance.

FILE AN SMS REPORT
- FOM 3.4 Submit SMS Report as soon as possible from the end of the duty period.
- If you have questions or concerns, contact your Director of Safety.
